Dear Parents/Players,
The CYSA Board of Directors would like to thank you for supporting our organization. We ask all of our members please take a few moments and fill out a coach evaluation. Your observations and comments are valuable, very important and all feedback is confidential, unless you choose to be contacted by a board member for further discussion.
Your survey response is important to the Board and helps us not only to provide a good coaching staff for your child, the evaluations also help us to improve and target coaching development clinics as well.
